IFDA keeps an up to date version of electronic fillable forms for your ease of use. Those forms can all be found and downloaded for your use from this page. However, we understand that many of you would also like to continue to use the duplicate and triplicate copy contracts in paper form. Should you wish to order those from IFDA, please call our office and speak to staff about placing them in the mail to you. This is a FREE service to our members. Non-members may order forms in packs of 25 for $25.00 per type of contract by contacting the office via email at Admin@iafda.org or by phone at 515-270-0130.
